Un nuevo medicamento para la esclerosis múltiple, basado en el principio activo alemtuzumab, permite sustituir el actual tratamiento permanente y continuo por solo dos dosis en dos años, por lo que el 70 % de los afectados no necesitará más fármacos.

Este tratamiento retrasa las recaídas y mejora la disfunción neurológica, según dos estudios sobre la repercusión del medicamento en la esclerosis múltiple.

Estos resultados han sido presentados por el jefe del Servicio de Neurología de los hospitales universitarios Regional de Málaga y Virgen de la Victoria de Málaga, Óscar Fernández; por el jefe del Servicio de Neuroinmunología del Hospital Vall d’Hebron de Barcelona, Xavier Montalban, y por el director de la Unidad de Esclerosis Múltiple del Hospital Universitario Virgen de Macarena de Sevilla, Guillermo Izquierdo.

El medicamento, que la farmacéutica Genzyme comercializa como «Lemtrada», supondrá una nueva respuesta para pacientes con manifestaciones clínicas de una enfermedad que afecta a 45.000 personas en España, según ha explicado Salvador García, director médico de este laboratorio. 

Cada año se producen 2.300 nuevos casos de esclerosis múltiple, patología crónica del sistema nervioso que provoca trastornos motores y sensitivos. En pacientes con estas dificultades, frente a los tratamientos diarios y semanales, el alemtuzumab se les administra por vía intravenosa durante cinco días, en una primera fase, y durante tres, doce meses después. Tras estas dos dosis, cerca de tres cuartas partes de los pacientes no necesitarán nuevos fármacos, debido a la capacidad del anticuerpo monoclonal para bloquear las células vinculadas a la enfermedad y regenerar el sistema inmunológico.

Esta acción permite retrasar las tasas de recaídas anuales, de 3 a 7 años en los pacientes nuevos, y de 2 a 4 años en casos en los que ya se había utilizado otro fármaco.

Según los estudios, en la Escala Ampliada del Estado de Discapacidad (EDSS) de Kurtzke, que evalúa la condición neurológica entre 0 y 1, el doctor Fernández ha explicado que, por ejemplo, si se diagnóstica el paciente en un nivel dos, el nuevo fármaco puede reducir su disfunción en 0,75 puntos.

Ello no ocurre siempre, y cuando sucede, «no decimos que están curados sino que están libres de la actividad de la enfermedad», ha apuntado por su parte el doctor Guillermo Izquierdo.

Estos efectos positivos se han comprobado en pacientes con la enfermedad activa y «cuanto antes mejor, pues no somos buenos en frenarla en épocas tardías», ha indicado el doctor Montalbán.

En todo caso, el director médico de Genzyme ha mostrado su esperanza por los nuevos resultados, pero también prudencia, por ser un medicamento sujeto a seguimiento y notificaciones de los profesionales, para aumentar la información sobre su seguridad. EFE.
