Numerosos estudios publicados en revistas científicas en los últimos años han comenzado a otorgar importancia a lo que la revista Science considera uno de los 10 descubrimientos más importantes de esta década (Science, 17 diciembre 2010): el microbioma o flora intestinal.

 

Estos estudios parecen indicar que la flora bacteriana intestinal podría tener un papel muy importante en la génesis y la progresión de la esclerosis múltiple.

A través de nuestra dieta, modificaríamos nuestra flora intestinal, y esta modificación daría lugar, en personas genéticamente predispuestas, a la aparición de diversas enfermedades. La maduración del sistema inmunitario y endocrino está influida por la colonización de bacterias. La flora intestinal protege al hombre de enfermedades como el asma, la alergia y las enteropatías inflamatorias crónicas. Está relacionada con múltiples enfermedades: autoinmunes e inflamatorias (cáncer, la enfermedad de Crohn, osteoartritis, el síndrome de fatiga crónica, la enfermedad inflamatoria intestinal, la esclerosis múltiple) y trastornos de salud mentales, que ahora son considerados como comunes, como la depresión, la ansiedad, el TDAH y el autismo.

 

¿Qué es el microbioma?

Los dos kilos de bacterias con los que convivimos habitualmente se conocen como microbiota. El ADN de esas bacterias se conoce como microbioma. Se encuentran en la piel y en la vagina femenina, pero principalmente colonizan el aparato digestivo, desde la boca hasta el ano. En total son más de 100 billones de bacterias, que constituyen tres enterotipos diferentes en el ser humano.

 

¿Cuáles son sus funciones?

Las bacterias intestinales son responsables de metabolizar los residuos no digeribles de la dieta, el moco endógeno y los detritus celulares; producen vitaminas: K, B12, biotina y ácido fólico; sintetizan aminoácidos a partir del amoniaco y la urea; nos protegen de la implantación de bacterias externas y desempeñan un papel esencial en el desarrollo del sistema inmunitario, siendo muy importantes en la conformación del estado de inmunotolerancia activa mediado por células T reguladoras. Por su importancia, se considera que constituyen un órgano en sí mismas. Cuando estas bacterias se alteran se produce lo que conocemos como disbiosis. Su importancia es tal que ya existe un proyecto internacional que busca secuenciar su genoma, The Human Microbiome Project (http://commonfund.nih.gov/hmp/index), para poder relacionarlo con la aparición de distintas enfermedades humanas, entre ellas la esclerosis múltiple.

 

Relación microbioma-dieta-esclerosis múltiple.

La alteración de esta flora intestinal, a través del estilo de vida y la dieta, podría dar lugar a la aparición de diversas enfermedades en función del perfil genético de la persona. Dietas incorrectas producirían modificaciones en la flora intestinal. A su vez, estas modificaciones producirían un aumento en la permeabilidad intestinal y, a partir de ello, grupos de sustancias que normalmente no entrarían en nuestro organismo, pasarían, dando lugar a distintas enfermedades. A este respecto, el Dr. Jean Seignalet, en su libro ‘Alimentación, la tercera medicina‘, señalaba la existencia de tres posibles grupos de personas que desarrollarían tres posibles grupos de enfermedades:

– Personas en las que, por su genética, su organismo lucha a toda costa por eliminar todas las toxinas que la disbiosis haya permitido entrar: son las enfermedades de eliminación (colon irritable, enfermedad de Crohn, psoriasis, asma bronquial…).

– Personas en las que su organismo tiende a acumular estas toxinas para que no hagan daño al circular: producirían enfermedades por acumulación (fibromialgia, síndrome de sensibilidad química múltiple, artrosis, cáncer, Alzheimer…).

– Personas en las que su organismo reacciona contra estas toxinas a través de su sistema inmune, pero al estar éste alterado debido a la disbiosis, se confunde y ataca también a sus propios órganos. Se producirían las enfermedades autoinmunes (esclerosis múltiple, artritis reumatoide…).

– Yo añadiría un cuarto grupo: personas que han heredado una genética envidiable, que tendrán una vida longeva y con salud. Es el grupo en el que todos querríamos estar, pero por desgracia, es escaso. Así que mientras pertenezcamos a uno de los otros tres grupos, ya que no podemos modificar nuestra genética (por el momento), lo que sí podemos modificar son algunos factores ambientales y sobre todo, la dieta.

En el diseño de una dieta saludable hay que tener en cuenta que vamos a chocar de pleno con muchos intereses económicos. Así pues, ¿Qué alimentos son más susceptibles de producir alteraciones en el microbioma? Comenzaremos por lo que ya nadie discute: hay que evitar totalmente las grasas trans, los ácidos grasos saturados, los aditivos, conservantes y colorantes, y los productos contaminados con pesticidas, fungicidas y herbicidas, y los alimentos transgénicos. Hay que recordar respecto a estos últimos que la legislación no obliga a especificar su procedencia en el etiquetado si no supera una cierta proporción; en caso de etiquetarse aparece, lo más pequeño posible, como OMG (organismo modificado genéticamente), y no hay obligatoriedad de etiquetarlo cuando son animales alimentados con piensos transgénicos. Son utilizados en la alimentación animal y por tanto introducidos en la cadena alimentaria. Hay que eliminar la leche: los mamíferos sólo necesitan la leche, de su misma especie, mientras son lactantes. A las vacas se las alimenta con piensos de maíz y soja, lo que altera la proporción de omega 3 (antinflamatorio) / omega 6 (inflamatorio), que idealmente debería ser de 1/1 pasa a 1/15-1/40. Además, para producir más leche se les suministra estrógenos que se ingieren con la leche y que se han relacionado con la aparición de cáncer de mama en la mujer; se les suministra BST, somatotropina bovina, para que crezcan más, y esta hormona se ha relacionado con crecimientos tumorales, y finalmente, para que las mamas de estos pobres animales permanentemente conectados a extractores de leche no se infecten, se les suministra antibióticos que están dando lugar a la aparición de las superbacterias, resistentes a todo. Por tanto, hay que eliminar la leche de la dieta. No hay problema con el calcio: se pueden consumir suplementos que aporten la cantidad diaria necesitada, unos 600 mg (Feskanich et al, 2003) y encontrarlos en otros alimentos (una lata de sardinas contiene 400 mg). Hay que suprimir el azúcar refinado que puede sustituirse por estevia, panela o sirope de ágave. Hay que suprimir las harinas refinadas y consumirlas integrales. Y atención al gluten. Recientemente se ha descrito la Sensibilidad al Gluten No Celiaca (trabajos del Dr. Alessio Fasano). Se calcula que entre el 5-6% de la población puede presentarlo. El gluten podría estimular una molécula intestinal llamada “zonulin”. Esta molécula podría hacer permeable al intestino al ingerir gluten en personas genéticamente susceptibles, pero no se producirían los síntomas de la enfermedad celiaca, sino que el intestino se inflamaría y se volvería permeable. Se recomienda disminuir el consumo de carne roja y evitar los productos de carne procesados. También se debe disminuir el consumo de productos curados, ahumados o en salazón, y el cocinarlos a altas temperaturas (fritos), siendo preferibles como métodos de cocción hervir o cocer al vapor. Respecto al microondas, Joseph Mercola ha comunicado que la estructura química de los alimentos cambia cuando se introducen en el microondas, con resultados inciertos para la salud.

Además, en la medida de lo posible, debemos intentar aumentar el consumo de alimentos saludables: té verde, aceite de oliva virgen de primera presión en frío, cúrcuma (curry), coles, repollos, brócoli, frambuesas y arándanos, nueces, fruta de hueso grande, kéfir, quinoa, ajo y chocolate negro, entre otros. Respecto a la suplementación con probióticos, imaginemos la colonización del intestino por bacterias vía oral como el desembarco de Normandía. Para sobrevivir al fuego enemigo (los ácidos del estómago) tienen que desembarcar por millones para que alguno llegue vivo al intestino,  y ningún yogur las lleva en esa cantidad. Así que olvidémonos de ‘yogures maravillosos‘. Lo mejor es un suplemento de probióticos que se adquiere en farmacias. Requisitos: que lleve tantas cepas bacterianas diferentes como sea posible (5 mejor que 2, y 10 mejor que 5) y en cantidades superiores a 109 UFC/g (hablamos de mil millones de bacterias por cepa).
